All required forms can be completed on athleticclearance.com. All physicals must be updated and completed on the physical form attached below. Please select the 2026-2027 school year along with the sport that you are interested in trying out for.

Athletic Release Form - Allows an athlete to leave an event with a parent or guardian. This form must be submitted 24 hours before the athletic event. Student/athletes will only be released to persons listed on their Emergency cards.
